March 2013 Giveaway: Medical Binder Kit

It's almost spring in many countries and it's the season where a lot of people choose to clean up and get ready for summer! Last week I wrote about the importance of keeping good health and medical records, so this month's giveaway is my Medical Binder kit! 





This set of organizing worksheets will help to "spring clean" for health as it can be used to make a Medical Binder for you and your family. I know, I know...thinking about health and medical stuff is no fun, but in the long run it's worth it.




Included in the set are the following:

COVER PAGE - Personalized with your family name or whatever name you'd like. You can also choose what color you want for the cover. Color choices are blue, yellow, green, grey, orange, or pink.

MEDICAL HISTORY (2 pages) - This complete form will allow you to note important information about your health including blood type, a record of immunizations, major illnesses, surgeries, etc.

FAMILY HEALTH HISTORY - This is where you can write down any information your doctor should know about your family health history.

MEDICAL CONTACTS - Create a contact list for important medical professionals like your doctor, OB, dentist, emergency numbers, etc.

DENTAL RECORDS - Keep track of all dental treatments on this sheet and you'll never forget a tooth cleaning appointment!

DOCTOR VISITS - Keep notes on all your visits to any doctor. Also great if you have a lot of different doctors.

PRESCRIPTION TRACKER - Whenever you're prescribed a particular medicine, write down the name, dosage, frequency, and notes.

LAB TESTS & RESULTS - Often times, after a visit to the doctor you may have to undergo various medical tests to figure out what is causing a particular illness or symptom. This sheet is where you would write the type of tests and the results of them.

SYMPTOMS TRACKER - If you have any sort of allergy or food intolerance, then you'll know the importance of keeping track of any symptoms you experience, to find out what may be causing your discomfort.

  4. My health goals are to keep my four kids sick free for longer then a week. I have four girls 8, 6, an 2 yr olds twins. I have to take at least one to the dr every other week. Last week they was all sick and I had to buy a note book to keep up with med and tenps so this would really help me out a lot! Thank you for the chance to win!

  9. My health goals are to take vitamins daily with my fiancé, and finally get our lives organized. I had to help pass on info during his recent surgeries and there is so much I had to track down, blood type and allergies being among some of the stuff I didn't know about him. Before we have kids I want to be more organized!
